Each item must be completed and turned in for you application to be processed. If you have any questions about the forms or procedures, please contact Tania Tucker Kraus, Senior Coordinator, Office of International Programs at trtucker@fau.edu or 561-297-3227. Please note these procedures only pertain to non-degree seeking students, students planning on getting a degree from FAU must complete the regular International Admissions Application. If you want to live on campus, please contact Tania Tucker Kraus immediately, as the procedures for applying for on-campus housing as a non-degree student is different that what is listed on the housing website. Mark off each item as it is completed to help you keep track of your paperwork. _____1. Complete the Non-degree Application. Do not complete the student identification number, as that will be assigned to you at a later date. _____2. Complete the Residency Classification Form. _____ 3. Download (http://www.shs.fau.edu/Acrobat%20Files/Immunization_Form.pdf) and complete the Student Health Form. Your doctor needs to complete section B with the requested information, sign the form, and provide the office stamp. You need to complete section C and sign the form. Pay particular attention to immunization requirements. In particular the first injection needs to be given after the students first birthday and the second injection needs to be given at least 30 days after the first. If you receive your injection before your first birthday or less than 30 days apart you will need to get another and provide documentation of that injection. _____ 4.Provide funding information. All financial documentation needs to be provided in English and in U.S. Dollars. Students need to demonstrate that they have a minimum of $30,242.80 for the year (30 credits) and $12,601.16 for one semester. If a student has a full tuition waiver, the bank letter can be lowered to $1,500.00 per month. Financial documents can be in the form of a bank letter or bank statement. If you are being sponsored or support is coming from a scholarship, please provide the original letter of award. _____ 5. Download the Certificate of Financial Responsibility Form from http://www.fau.edu/academic/admissions/FinCertFormFCF06.pdf and complete it. |
